NorthLakes Community Clinic
NorthLakes Community Clinic is a Federally Qualified Community Health Center that operates clinics throughout northern Wisconsin spanning across 20,000 square miles.
Our Hurley Clinic currently offers Behavioral Health, Chiropractic, Dental, Medical, Nutrition Therapy (Virtual), Patient Supports, and Pediatric Occupational Therapy.
We provide a variety of services in our clinics based on the needs of the communities served. All people are welcome to receive care regardless of their insurance status and ability to pay. For those who qualify, we offer a Sliding Fee Scale, which is based on income.
We are deeply involved in our communities. We offer Community Outreach services, which include the largest Dental Outreach Program in Wisconsin as well as School-Based Behavioral Health. We also take Dental services to nursing homes and work with WIC (Women, Infants, and Children). Since our opening in 2008, we aim to fill the gaps in healthcare and fulfill our vision of healthy, prosperous, engaged communities where everyone thrives.
As a Community Health Center and National Health Service Corporation, we promise to :
● Serve all patients.
● Offer a Sliding Fee Scale for patients who qualify.
● Not deny services based on a person’s race, color, sex, national origin, disability, religion, sexual orientation, or ability to pay.
● Accept private insurance, BadgerCare, Michigan Medicaid, or those with no insurance.
We are governed by a patient-majority board that meets monthly.
